All additional formats to be saved - emails, excel

Allow for other formats of document to be saved to the system including excel and emails - currently having to save documents in these formats into another format to be able to save them to an applicants record.

All values should appear on reports even if there have been 0 responses.

When reporting from the system, if there have been no responses to a question this should still appear on the report even if it is 0.
